We strongly advise you to save the disclaimer page as an HTML document on your computer in case you have printing problems.
The disclaimer should fill most of a single page. To print, just click "Print" in the top frame of your browser (e.g. Netscape or Internet Explorer). You'll need to print, sign, and mail in three copies. In the past, some users reported minor difficulties while printing out the confirmation page, as follows:
1) The print is too small and
2) The print is too large and the text spills onto a second page.
3) The page prints sideways.
If you are experiencing one of these problems problem, try cutting and pasting the text into a word processing document and formatting it correctly there. Here are some general instructions for a PC using Internet Explorer and Microsoft Word. The commands might have slightly different names in the software programs you use or might be different if you are using a Mac.
a) Click the "Edit" button along the top frame of your browser.
b) Click "Select All."
c) Click the "Edit" button a second time and then click "Copy."
d) Shrink or close the browser (to shrink, use the "_" button in the very top right-hand corner of your screen).
e) Open a word processing program (e.g. WordPerfect or Microsoft Word) and make sure you have a new document.
f) In the word processing program, click "Edit" and then "Paste."
f) Make sure the entire text is in 10 point type. If it doesn't fit on a single page (or is too small), adjust the font size accordingly. If your printer is set to print "Landscape" (sideways), you'll need to change this to "Portrait" (vertical). Check under File > Page Setup > Paper Size
g) Print three copies of the page.
